We strengthen community-based support through structured coordination, advocacy, and person-centered service navigation. Our goal is to help individuals understand their options, make informed decisions, and connect with the resources needed to support their personal goals.

Through Support Brokerage and Care Coordination services, Twin Pathways Support assists individuals in navigating complex health and human service systems while promoting choice, independence, and continuity of care.

Our Services

Support Brokerage (DDA Self-Directed Services)

We support individuals enrolled in self-directed service programs by assisting with service planning, provider selection, and understanding available supports. Our role is to promote informed choice and effective use of approved services.

Care Coordination Support

We assist individuals and families in coordinating services across multiple providers and systems of care. This includes helping reduce fragmentation and improve continuity of support.

Resource Navigation & Advocacy

We support individuals in identifying and accessing community-based resources, completing applications, and connecting to appropriate services and supports.

Care-Bridge Model (In Development)

The Care-Bridge Model is an emerging care coordination framework focused on strengthening engagement, improving service continuity, and enhancing system navigation outcomes for individuals receiving support services.
